We have ten locations with the main office in Orangeburg. Here you will find caring doctors and nurses who strive to give you the best possible health care, in a friendly, inviting atmosphere. We take the time to get to know you and truly listen. Nothing makes us happier than keeping you and your family healthy! Our goal is to improve the health of the people of the community and region.

Many of our patients have been with our practice since they were young. We love seeing our patients grow up through the years. Above all, we take pride in offering the best possible health care. Our History

Family Health Centers, Inc. (FHC) is a federally funded community health center serving the medically underserved residents of Orangeburg, Bamberg, Calhoun, and Upper Dorchester counties in South Carolina. The health center was organized in order to provide high quality, comprehensive health care for the medically underserved. The organization has grown from one health center which is the main site on Magnolia Street, as well as comprehensive primary care sites in Bamberg, Neeses, Denmark, St. George, Vance, and St. Matthews, an optometry and dental fully equipped medical mobile units. Services offered include, Adult Medicine, Pediatrics, OB/GYN, Podiatry, Dental, Health Education, X-ray, Laboratory, Pharmacy, Migrant Farm Worker Program and Behavioral/Social Health. FHC is one of the largest rural health centers in South Carolina. FHC is also accredited by The Joint Commission, a prestigious quality accreditation that must be earned and maintained.

Primary Care Medical Home

In 2012, the Family Health Centers, Inc., became the first FQHC to be accredited by the Joint Commission as a Primary Care Medical Home in the state of South Carolina.

In 1996, Family Health Centers, Inc. (FHC) was accredited by The Joint Commission (TJC), a prestigious healthcare accrediting agency. The Joint Commission verifies the health center is providing quality care and is compliant with nationally recognized standards and continuously makes efforts to improve the care and services it provides.

The Million Hearts Program

Our Million Hearts Program is designed to identify those who may be at risk for stroke and/or heart disease. The goal of this program is to include preventive strategies into your treatment plan that will reduce your risk of having a stroke and/or heart disease.

Accountable Care Organization

Family Health Centers, Inc., also participates in a Medicare Shared Savings Program called Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals, and/or other health care providers working together with Medicare to give you better, more coordinated service and health care.

Healthy Outcomes Plan (HOP)

Healthy Outcomes Plan (HOP) Program is a state funded program and the Outreach and Enrollment Coordinator works in conjunction with the Regional Medical Center's HOP Coordinator to identify patients who are chronically ill and uninsured. Family Health Centers, Inc., schedules initial appointments for screenings and medical home assignments. HOP requires the patient to complete enroll- ment forms and the determination to be accepted into the program is handled by the Hospital HOP Coordinator. Eligible patients are reviewed every six months for continued participation in the program.

for Health IT Grant Program

Family Health Centers, Inc., recently received a grant from the U.S. Department of Health and Human Services for Health IT to further advance healthcare reform goals, including much-needed up-skilling for healthcare workers and facilities in health IT related fields. The U.S. Department of Health and Human Services (HHS) is the U.S. government's principal agency for protecting the health of all Americans and providing essential human services, especially for those who are least able to help themselves. The mission of HHS is to enhance the health and well-being of Americans by providing for effective health and human services and by fostering sound, sustained advances in the sciences underlying medicine, public health, and social services.

Screening, Brief Intervention, SBIR
& Referral to Treatment

FHC provides Screening, Brief Intervention, and Referral to Treatment (SBIRT) to all our Medicare patients, SBIRT is an approach to the delivery of early intervention and treatment to people with substance use disorders and those at risk of developing these disorders.

Dual Eligible

You may qualify for Medicaid benefits upon reaching age 65, we will assist you with applying for Medicaid. If you do qualify for Medicaid, you may not have to pay a co-payment/or pay a very small amount for health care.
