LPN Site Supervisor - Denmark:
Qualifications: Requires successful completion of an accredited licensed practical nurse program, plus one year of nursing experience in a healthcare environment, or any equivalent combination of training and experience which provides the required skills, knowledge, and abilities. Must possess state licensure as a Licensed Practical Nurse; must possess current CPR certification. Must possess a valid South Carolina driver's license.
General Statement of Job: Under occasional supervision, supervises and participates in the provision of medical support to facilitate the assessment, diagnosis and treatment of Family Health Center patients at assigned site. Work involves coordinating and supervising the work of subordinate nursing and support staff, providing direct patient care, maintaining accurate and complete patient charts, and performing related functions as required. Reports to the Site Manager.
LPN - Float:
Qualifications: Requires completion of an accredited License Practical Nursing program. One year of nursing experience in an outpatient healthcare environment is preferred. Must possess state licensure as a Licensed Practical Nurse. Must possess current CPR certification. Must possess a valid South Carolina driver's license.
General Statement of Job: Under occasional supervision, provides medical support within the scope of practice for an LPN to facilitate the assessment, diagnosis and treatment of Family Health Center patients in assigned unit or site. Work involves conducting initial health assessment, providing direct patient care, maintaining accurate and complete patient charts, and performing related functions as required. Requires rotating to various units/sites. Reports to the DONQS.
Migrant Outreach Worker:
Qualifications: A High School diploma or GED with one year experience working with farm workers is preferred. Proficiency in reading, writing and speaking Spanish, as well as communicating effectively in English is required. Certification in Medical Translation in Spanish is also desired. A valid SC driver's license is required.
General Statement of Job: The Migrant Health Outreach Worker (MHOW) is primarily responsible for reaching out to migrant/seasonal farm workers (MSFWs), especially the Spanish speaking individuals. This is to be accomplished by utilizing the Family Health Centers' (FHC) 15-passenger van to visit area farms that are already in the FHC's network. Through these visits the Outreach Worker will identify new farms, encourage appropriate use of health care services by MSFWs, create awareness among them regarding services available at FHC. The MHOW will provide interpretation services for the Hispanic population presenting at FHC for services, as needed. The Migrant Health Outreach Worker will also perform other duties as assigned that are within the individual's skills and/or expertise.
